11.  INSURANCE AND MEDICAL EXPENSES.  The ________________________ is

ordered to maintain each minor child as a beneficiary of any medical,

dental, and optical insurance program that is now or may become available

through the _______________________’s employment and such coverage will be

continued so long as it is available through such employment.

     12.  UNINSURED HEALTH CARE EXPENSES OF CHILD(REN).  Any medical,

dental, orthodontic, and optical bills for the child(ren) that are not

covered by insurance will be paid for as follows:

     ( )  one-half (1/2) by each of the parties.

     ( )  by the plaintiff.

     ( )  by the defendant.

     13.  CHILD(REN) TO BE BENEFICIARIES OF LIFE INSURANCE.  The

_________________________ will maintain each child of the parties as a

beneficiary of any and every life policy the _______________________ may

have through employment or otherwise until each child attains the age of

majority or otherwise becomes emancipated, whichever occurs first.

     14.  CHILD CARE EXPENSES.

equally by the parties.

     ( )  The net amount of all work-related child care expenses (actual
